BQ20Z45: CFETF false triggering in production

I currently have a design implementing the BQ20Z45-R1 with a few thousand units deployed in the field.  Customers have started to find that their batteries are failing prematurely and sending them back.  Upon testing the packs we have found that CFETF permanent failures have occurred in every case.  When it was first discovered we updated our .gg file settings surrounding FET failure to be 100mA of current detected for 20 seconds.  Even after this update we have found a few cases where the failures have occurred again.  I find it hard to believe that once the CFET shuts off we are having a continual current flow because the FETs are operating normally and are not damaged at all.  What direction could you give me for helping to find root cause?
