LM25066A: MOSFET burnt issue

Hi Team,

My customer encountered MOSFET burnt problem when he used LM25066A. The SCH as below, customer faced Q14 or Q10 sometimes burned when power on, please help to check below parts value of schematic if reliable or not, current limit setting is correct or not and which part is critical relative with Q14 & Q10 operation?
