LMZ10500: Failure Analysis

Customer found the defetive part after performance testing. From the decap screen shot, what would be the root cause? Voltage overstress or other possible reasons?

Let me look into this and see what the quality engineer thinks of this issue. If possible can you contact me at j-hua@ti.com with the schematic and pcb layout of your customer's system?I'd be good to know what conditions they have the system in to cause this.
