TPS54360: Damage Cause Analysis Request

After the AC220V is transformed and rectified, the output DC26V-36V is connected to the input of TPS54360. TPS54360 output adjustable voltage 12V-24V, maximum input current 1A, connected to inductive load. Our KMM customer (Guilin Woodpecke) carries out functional testing before aging testing in the production process. There is a 5% damage rate in the aging testing process. The damaged circuit was replaced with a new TPS54360 and returned to normal operation. We tested the damaged IC's PIN2 (VIN) and PIN5 (FB) short circuit to GND. The circuit diagram is attached. Could you help us to analyze the cause of the damage? I   n addition, how to improve the circuit? Do you need to add some protective devices?
