LM5060: LM5060 Gate Pin Issue

I saw similar questions at E2E, but found no solution.I have the same problem now. Here is my schematic

I have tested my MOS FET and confirmed that it is good.

I replaced the LM5060 with a new LM5060, and the circuit can work normally.It seems that there is something wrong with Gate Pin. It seems that no charge pump works.

Can someone help me solve this problem?I tried to produce 100 sets and found 3pcs defective, which has been bothering me for a long time.

    Is the failure happened during board bring up for the first time? Or after doing any specific test ?

    Please let me know if it is time0 failure or in the field?

    The internal circuit damage happens for EOS events. Do you expect them in your system ?

    What is the application use case?

    Have you used any TVS clamping diode at the input?

    Reply:YES.Normal power on test, and  INPUT Voltage is 20V.

    Reply: TVS and RC be used in the input.

    Please share me the TVS part number.

    If you would not expect failure due to overvoltage, I suggest to submit the unit for failure analysis.
