LM5117: CC+CV mode: With no load, input current ramps up and damages Q high side

Hello.  I made a design with the LM5117 in CC+CV mode according to the datasheets and spreadsheet calculators.  In a few prototypes, the input current draw ramps up very high even when there is no load and eventually damages the high transistor.  I presume it would fail faster if it wasn't for the long soft start.  The design uses an external Vcc supply of 11.5V. 

I'm hoping for a few pointers on where to start troubleshooting this issue.  My company policy doesn't let me post the schematic publicly - is there a way I can have troubleshooting support without publishing the schematic publicly?
