What changes would you recommend to the application circuit for 12VOUT application at 5A?

Do you have any recommendations on Si MOSFET p/n?

    Thank you for your interest in the UCC24612 SR controller.

    My recommendation would depend on consideration of additional details involving the 12-V, 5-A application.
    In particular, what is the maximum peak voltage impressed across the SR MOSFET when that FET is off?

    The MOSFET on the UCC24612-1EVM is a 150-V, 19-mR, BSC190N15NS3 from Infineon Technologies.
    As the EVM User Guide says, the EVM is designed for current levels up to 5A, and the 150-V rating makes it suitable for most typical 12-V flyback applications.
    If you applications is such a typical one, then I wouldn't recommend any changes to the EVM.

    However, if your application is not a typical off-line flyback, perhaps with a peak voltage stress close to or higher than 150 V, then on general principles, I'd recommend choosing a MOSFET with about 50 V of margin to the peak stress, and with on-resistance about the same: 19 mR +/- a few.
