TPS54560-Q1: Sometimes failing at power up, case blowing up at pin 2

I am using TPS54560QDDARQ1 and sometimes at power up, it is failing. The case blows apart (small chip out) where the pin 2 lead meets the case, or pin 2 melts away causing an open. Usually the circuit works fine. I can run it for days, fine. The load is 2, brushless DC fans at 24V, 1 amp each. Input voltage is 42V. I checked the 42V at turn on, it does not overshoot 42Vdc in the 10 or so tests I scoped. 

When it does fail, it pops immediately at power-up. It "pops" and then must be open circuit, because the chip does not further burn up.

I am using this part for 3 other regulators in the same design, from a rail derived from the same functional supply, and they do not fail.

(also, a shout out to WebBench, besides this intermittent issue, the design worked perfect on the first PCB spin)
