LMZ12001: Very high ripple on output voltage

I have a design using three of these simple switchers, and all of them have really high output ripple.  For instance the 1.8V rail goes from 2.2V to 1.5V.  I tried adding 100uF to the output and another 100uF to the input but that had no effect.  The load is pretty small just a single TI DSI converter and the bench top supply that is providing the input of 5V shows the whole system is only drawing 250mA so the load should be really low.

I'm not sure what I did wrong here.  I've attached a scope shot, schematics and a layout picture.  The scope shot was taken with a 500Mhz bandwidth passive probe and the gnd wire was less than 1/4".  Finally when there's no load the voltage is fairly stable.  What did I miss?
