TMP423: TMP423AIDCNT

I’m using a temperature monitor device from TI (TMP423AIDCNT) to read the temperature of multiple LTM4630A DC/DC from Linear Technology on my board as shown below. I’m getting erroneous temperature readings and was wondering if TI had any suggestions as to what the issue is?
