OPA4188 Pin Names

I am trying to use the OPA4188 Spice Model and using PSpice as my simulation. The model/part doesn't have any pin names (ie. Out, Vs, -Vs..). Its labeled as 1,2,3,4,5 and without knowing the pin names its hard to run simulation and get correct result.

    You can see the functional pin names by left-clicking on the model symbol and selecting "Enter Macro".
    This action opens a text editor and allows the user to see the informational header and the model netlist.
    The header gives information on the simulator and parameters supported by the model.

    For the OPA4188 model, the last line of the header give the functional pins names:
    * OPA4188 pin order  +IN  -IN  V+  V-  Vout 
    .SUBCKT OPA4188        1    2   3    4    5
