CC2640 : 4x4 package in self-design schematic and PCB. Error when download CC2640Stack-FlashROM. (Fatal Error: Failed to halt after bootloader)

I'm trying to download active application from IAR 7.40 to my self-design CC2640 layout (with PCB and SMD on it), but always get the error message when download Stack active application to CC2640.

The error message as below,

By following the step of Compiling and Download in "TI CC2640 Bluetooth low energy Software Developer's Guide" as,

    1. Build the Application project using Project -> Make

    2. Build the Stack project using Project -> Make

    3. Download the Application project using Project -> Download -> Download Active Application

    4. Download the Stack project using Project -> Download -> Download Active Application

The first 3 steps are OK without any errors, but dead at 4th step.

In SmartRF Flash Programmer 2, CC2640 can be successfully connected, erased, and read the memory data

as the screenshot below,

I have tried the self-design schematic with software in the previous version of CC2640 (ble_cc26xx_2_00_00-41482-alpha-0.8.2), and it works OK.

However, since I changed to the newer version of CC2640 (ble_cc26xx_2_01_01_44627), this error message keeps pop up when downloading Stack Project Active Application.

Did anyone met this error before?
