CC3135MOD: Cannot connect to Cisco AP

Somehow the CC3135MOD can no longer connect to the Cisco AP after a software update on their wireless LAN controller, based on the logs collected by the Cisco WLC, the client device (i.e. CC3135MOD) didn't send any association request after receiving the probe response. What could be the reasons and any suggestion on how we can debug this issue?
