CCS/CC3220: Flashing cc3220

Hello,

I am having serious problems flashing a cc3220 on  a custom custom board. I have followed the reference designs for the launchpad with few modifications. So far i have tried about 5 revisions on the cc3220S. cc3220sf and the ss3220MODA chips. I have tried flashing these things with CCS and it always fails. I have tried both with UART and with JTAG. When i run JTAG test connection all scans complete successfully. the error i am seeing is:

Error connecting to the target:
(Error -2131 @ 0x0)
Unable to access device register. Reset the device, and retry the operation. If error persists, confirm configuration, power-cycle the board, and/or try more reliable JTAG settings (e.g. lower TCLK).
(Emulation package 7.0.48.0)

I am open to any suggestions.
