I was told that this device must be verified as bad from this forum before I can get a replacement. It is clearly bad, no quality assurance from TI, I have wasted several hours and has cost me money trying to get this faulty board to work. A simple AOI or test at the end of manufacturing would have caught this. In fact, I've gotten a lot of prototype PCBAs before, never have I seen such poor quality all the way from the PCB to the assembly process.
