CCS/CC1350: Errors connecting XDS110 to custom board target

We are having trouble programming and debugging our custom CC1350 boards using Code Composer Studio version 8.0.0.00016 and Smart RF Studio 7 version 2.9.0. We are using the XDS110 tool on the CC1350 Launchpad with an external target.

Typically, when attempting to debug from CCS, we get the following error:

After some retries (typically two), the programming proceeds and we're left ready to debug.

Sometimes resuming in the debugger continues the execution of the program, sometimes it does not (and requires terminating the debug session and power cycling the MCU).

We've been through many of the forum topics on this "router subpath" error and have tried many things, including:

  1. checking and tweaking the board configuration
  2. checking the voltage levels
  3. flash erase
  4. downgrading and upgrading the XDS110 firmware
  5. changing USB cables
  6. changing computers

When attempting to use the SmartRF Studio program with this board, we get the following message most of the time. Very occasionally, it proceeds to the Device Control Panel, where all is well (able to select frequencies and data rates, use Continuous and Packet Tx/Rx).

The same XDS110 tool on the Launchpad is able to work with the CC1350 on the Launchpad (consistently able to program, debug, work with SmartRF Studio).

Does anyone have any ideas as to what is causing these behaviors? It seems to be specific to our non-Launchpad CC1350 design (same issues on 4 separate boards). It seems quite strange that things occasionally work.
