I suspect M2 Mosfet is damaged by voltage spikes during its turn off process.
If you are still using CSD17578Q3A as M2 Mosfet I would try to add gate resistor of around 100 Ohm, it should extend its turn off time and limit
voltage transients…

The recommended replacement for the CSD1755Q3A is the CSD17578Q3A.
The HIDRVx and LODRVx pins output up to 6V across the Gate-Source of the power MOSFETs. So MOSFETs with +/-10V to +/-20V rated VGS should be fine.
