We have a 48VIN application in the datasheet, it is definitely possible to use AEF with LM5149 and at 48V VIN.
50mA would be a worst case, I have measured ~15-25mA in typical AEF use cases.
But yes, the efficiency hit would be more significant…

15mA is about expected for this device.
You could improve Iq by placing D1, this will power VCC off of VOUT.
Just be sure the VOUT is in the range for external VCC (8V-13V).
Also consider LM5148 or LM5149…

The LM5148 is the same DC/DC controller as LM5149, where LM5149 uses Active EMI filtering in place of the NC pins.
However without active emi filter the devices are the same.
