Based on email you sent earlier today, we think this could be due to the fact you are using LMH1208 device versus in our measurement we are using LMH1228 device.

Yes, with the LMH0318, it is possible to use both OUT0+ and OUT0- polarities to route to separate 75-Ohm cables.
OUT1+ and OUT1- should only be used for 100-Ohm differential media, such as differential PCB traces or SFP+ copper attachments.
