I checked and the LMH0324EVM is no longer available. Please use the LMH1219EVM instead. The LMH1219EVM has a 75 ohm input and 50 ohm differential output.

As is stated in the LMH1219EVM user's guide, the LMH1219 has the above 4-level input configuration, The configuration pins are set up the same way for the LMH1218.
