LM1875: LM1875 bridging FFT problem

Part Number: LM1875

LM1875 is used for middle AMP. It can't work normal, U2 and U4 bridge FFT are prone to problems. But there is no damage when the IC is removed. Attached is the design data. Is there any problem to solve the LM1875 bridging FFT problem?
