HI Roy,
Attached is the revised simulation file. I used two voltage controlled voltage sources to generate the fully-differential input signal. You can set the VICM supply to the required input common-mode voltage.

In this case, I performed a transient simulation using a 1kHz source; but you will need to change the frequency per your signal requirements; I only copied your circuit. If you need to determine the small-signal BW frequency response, you can perform an AC small-analysis.
You can perform a transient simulation from the top menu, as shown below:
