NE5532: NE5532 MEX ASO Issue

Can you tell us the circuit's supply rails? Please also provide me with schematic. It is not clear what you have.   

Also, the input Vicm voltage is from ±12V (min) from ±15Vdc rails. Please make sure that you are not exceed the design limits, including capacitive load at the op amp's output without compensation.
