Also, I noticed that U1B is unused. We recommend that the unused amplifier should be placed in a follower configuration to keep the amplifier output in a linear operating range. For more details, see the following:

Thank you for the photo of the failed unit, there is clearly a lot of current flowing through the part to cause it to heat up enough to melt the package.
Given that the part is failing as soon as the power supplies turn on, this could be an EOS issue, but given that I do not see any obvious issues with the schematic and that this design has been in production for five years, I'm inclined to think this is isolated to the device, but more information is needed.
When the power supplies are turned on, what is connected to the output of the amplifier (what is the load being driven in the circuit downstream of the amplifier in normal operation)?
How many devices in this batch have exhibited this issue (failure rate)?
Have you performed an A-B-A swap with an undamaged unit in the batch exhibiting the issue (2527)? This procedure is outlined here:
