How to implement long distance (more than 100M) digital audio transmission via RJ-45 CAT.5 cable?

A digital interface transmitter such as the DIT4192 could be used with an external LVDS driver. The interface forums may be able to provide more info in this department, but a starting point may be the RJ45 to SMA adapter board linked here (http://www.ti.com/lit/ug/snlu041/snlu041.pdf) which features the DS91C/D176 (http://www.ti.com/lit/ds/symlink/ds91c176.pdf).
