PCM2706 based USB DAC not recognized by Windows-XP

Dear Sir / Madam,

I have constructed a USB Sound Card using IC PCM2706 and as per the circuit diagram of the Datasheet of PCM2706/2707

Page 29, Figure 33 ( Typical Circuit Connection 2 (Example of Remote Headphone) ) 

But when I insert it in the PC USB Port running Win-XP it shows "Device Not Recognized" error message.

I have checked & rechecked the connections as per the diagram of the datasheet but the problem persists.

Hence I request you to please suggest and guide in this matter.
