TPA3140D2 dont work?

I am using TPA3140D2 in my PCB and connect it like the typical schematic in PBTL mode (Figure 26. Stereo Class-D Amplifier with PBTL Output and Single-Ended Input with Spread Spectrum Modulation).

But the voltage after 10k to VCC from nFAULT and nRESET is approximately 100mV - 1V in cycling, so I think it makes my PCB dont work (nFAULT is always ~0V, i think).

And TPA3140D2 is too hot, too.

I am using 12V power supply.

Anyone can help me how to solve this problem please, and why nFAULT is always ~0V, should I dont connect this pin to any nets.
