I found somewhere in this forum that your TPA3250D2's pins seem to be different with typical app.
But I thing that the app schem is for your TPA3251D2.
The question here is the pin INPUT_A and INPUT_B.
I thing INPUT_A should be 18 and INPUT_B should be 17.
Is it correct?
