TPA6141A2: The failure of TPA6141

Hi

In the Bluetooth noise-canceling headset project, I used two TPA6141.One is used for amplification of audio signals and the other to eliminate noise signal amplification.In a small batch of tests, the TPA6141 used for audio signal amplification always be damaged.The specific performance is the left or right sound track is noisy or silent, replace the damaged TPA6141, fault clearance.The schematic diagram of this part is shown in the attachment. Please help to analyze whether there is any need to adjust the schematic diagram. Thank you.

    I don't see any evident error in the schematic above. What is VBAT voltage?
    Do you have any more details about how and when the issue happens?

    The supply voltage is 3.7V, range of the input signal is 3.3V~4.2V, output load are two 32Ω trumpets.

    This project is an old project upgrade.The only upgrade point is the voice languige‘s type,which is pure software category. The hardware part has not changed.The old project has been continuously shipped more than one year, and there is no such abnormality feedback in the market.
    could TI help us analyze the failure samples?

    You should take into account that input voltage maximum values (given in Absolute Maximum Ratings) go from –0.3 V to HPVDD +0.3 V.
    HPVDD is set by the internal DC/DC converter, which maximum (also from Abs Max Ratings) is 2V. The input signal range you mention (3.3V~4.2V) is out of the specified max.
