TPA3118D2: Fault issue

Hello

                 Below is customer schematics and test waveform. Customer test some board but it happens 3pcs fail board and other units are good. The fail unit looks happen on output L for Woofer speaker that while power up. Then TPA3118D2 of fail units will happen fault then shut down output L to drive. Please refer to below test waveform 

                 Why fail unit happen fault of TPA3118D2 and others unit good to work on woofer speaker? If any suggestion for this issue? Thank you.

    Hi Patrick,
    Looks like FAULT is triggered and this pin is driven to Low voltage and then the device gets into shutdown(SDZ also in low voltage). The FAULT could be OC, DC or OT. Is there any analog input signal when the issue occur? If yes, can you measure the temperature on the AMP and estimate the output current on the woofer channel? Please try to short AMP_RIN and AMP_LIN to GND to see if the issue is still there.
