    After review of schematic and layout below are some comments:
    - I do not see input coupling capacitors, either on the schematic or the layout. I would recommend to place a series coupling capacitor at each input of the device, close to the IC. These capacitors remove any DC component from the input signal and also allow it to be properly biased for best performance.
    - Other than that the rest of the design looks good.
