TPA3255: TPA3255_OTW Issue

I have built a copy of the EVM board that is identical other than the power is fed from a PVDD supply (currently 40V)  and a separate 18V creating the 15V, 12V and 3.3V. All these supply are up and running and the TPA3255DDVR held in reset when powered. As soon as the reset is removed the OTW pin goes low, Fault pin flashes once and stay high. There is little ie less than 100mA being drawn from the 40V supply so I don't believe this can be an instant over temp condition. Is there anything else that can cause this pin to go low.

    I have found the issue, I was miss led by the data sheet that showed this to be a over temp warning. The issues was I had left out  the interconnect capacitors fro the buffer amp to the TPA3255 inputs, so they were seeing 6V DC from the amps, adding these caps and the amp started working.

    Not sure why I did not get a overload or somehow miss interpreting the info.
