TPA3221: Still problems with power up after reset

i have the same problems described in the thread "TPA3221: Issue that PWM switching doesn't start after /RESET is released", but as they discussed over private messages in the end i cannot see their solution. Can you please tell me how you solved the problem in the end or what the problem was caused by?

Decription of our problem:

TPA3221 is feed with 30V PVDD and 5V(external) with pullup resistor to the RESET pin. TPA3221 is working fine. We use a push button to pull the RESET low. When RESET is pulled low, FAULT goes also low (?datasheet is telling that FAULT is forced to be high when reset is applied). When RESET is released, FAULT stays low and the TPA3221 will not start again. When MUTE is applied (like recommended in the datasheet using 1k resistor and a push button) FAULT goes high. But when MUTE is released, FAULT goes low again.

Same behavior comes up up, when TPA3221 is running and MUTE is applied =>FAULT goes low and TPA3221 stops working and will not come back.

We designed our pcb to use the internal LDO. But RESET is feed by the external 5V.
