Given your condition, 18W is just on the boundary of voltage clipping. 12*12/2/4=18W. In this case, it's not limited by the device capability but the PVDD voltage.
To make sure you can meet the spec, I think you need to increase PVDD voltage or use lower resistance speakers.

I can understand V^2 = 12*12, and load is4 ohms.
but I cannot understand your formula, what is '2' mean?

It comes from the Vrms^2 = Vp^2/2. In BTL, the Vp=PVDD.
