TPA3123D2: Audio amp continues to have Thermal Shut-Down Issues

We have found that the amplifier will get HOT following power-ON cycle without any audio being passed through the part.

It seems to be in some unstable state with it consuming an extra 2-Watts of heat without audio.

When cooled down with freeze spray, it recovers and will operate normal for a while.

These are on units we receive back under RMA. 

The issue is not huge but we probably get about 20 RMAs with the problem per year out of 100 units shipped.

We would like to continue using this amplifier on next generation product but would like to resolve this before proceeding.

We have implemented changes that were thought to greatly resolve this issue under a previous TI thread ticket but the problem still persists.
