ADS54J69EVM: FT245R EEPROM Accidentally Overwritten

I have been using the ADS54J69EVM for a couple of months now without problems.

Recently I ordered a DAC5681EVM and was unable to get it working. However in trying to get it to work I used the TI USBSerializer software to overwrite its EEPROM.

The ADS54J69EVM must have been connected at the same time, resulting in its EEPROM being overwritten with the DAC5681(2)EVM values.

This explains why my ADS54J69EVM shows up as a DAC5682 - see below.

Can someone share with me the correct EEPROM values for the ADS54J69EVM and a way of flashing them - I don't want to have to buy another board.
