ADS1293: IEC60601-2-47 Test Circuit 300mV offset reference to ?

I am in the process of doing the IEC-60601-2-47.

I am trying to apply the 300mv offset to the ECG inputs via the S3 switch in the test circuit. Does anyone know what this 300mV is referenced to. It just shows the recorder.

Is it the recorder ground, the recorder Enclosure, Earth ground, The RL reference which is already connected to P^?  My recorded is a battery powered device with a plastic enclosure. The test is trying to apply 300mv offset to all the leads, but not sure what the offset is referenced to. 

  • I found the answer in an ECG simulator manual. The standard was unclear, but this shows the voltage is in series with the positive lead. I used a coin cell battery with a 0.3 volt circuit to put inline with the ECG. This keeps the circuit small and the noise very low. I don't have an expensive simulator to use, so I built my own test circuit. It seems to be working pretty well.
