DAC81404EVM: Only one channel appears to work.

I am using the DAC81404EVM and following the user's guide https://www.ti.com/lit/ug/slau912a/slau912a.pdf. All jumpers are in their default position. AVDD is powered by an external 5V. In the high level configuration page of the GUI, I am able to enable the internal reference voltage and power up/down channel A and set it to a desired value. However, when I try to power up any of the other channels, it automatically jumps to 5V. Changing the data value does not change the voltage. Does this indicate the device is damaged? Or is there another possible explanation?
