ECG cable and ADS1298 only

I got some questions about the aquisition of the ECG signal.

I would like to get an ECG standard (12 derivations or less), in order to make an holter prototype.

I studied how to transmit data from ADS1298 to a microcontroler. But i need your help concerning the real ecg data acquisition (on me for example, with an ECG cable or independants cables).

1) It is possible to connect electrods directly to the ADS1298 (like in your evaluation kit for example with a DB15 connector but just with the component ADS1298, or directly on the analog inputs pins) without intermediary components ? 

I think i need voltage for passiv electrods, no ? maybe just le potential from the heart...

2) In the case of a DB15 connector with ECG cable on me:

I don't really understand how you can deliver a standard ecg (12 derivations) with only 8 analog inputs, i read that we need 10 electrods so 10 analogs inputs to do it ...

Is there a converter into the cable to pass from 10 to 8 ? (i believe the 2 last derivations are calculated from the others)

    1)  You would need some protection circuitry. But, you might could start [Here].

    2) This Wikipedia link explains the derivations, quite well. Mainly, "Leads" are derived from multiple electrodes. On our EVMs, we make this 'easier' by use of differential inputs.
