I am using the app note from TI found at the link below.

The app note says that there are resources that go along with their proposed circuit, but the link just points me to a zip file with the original pdf file in it. Is there anywhere I can get these resources. I am building a spice model in LTSpice that should be equivalent to the one in the app-note, but it does not give the same output voltage swing and I am not sure why. I am also unsure why the model uses a VCVS instead of a CCVS to represent the CFA. Any help would be appreciated.
