Hello,

I wanted to know whether the DOUT in the ADS1291 is referenced to GND or given with respect to Vref. In addition, what does the internal reference (Vref = 2.4V) do?

    The DOUT signal and all of the digital signals will be referenced to digital ground on the ADS1291 (DGND). Vref of the ADS1291 is the reference voltage for the converter. A reference voltage is the scale by which an ADC converts analog voltages to digital codes. Table 10 in the ADS1291 data sheet relates the input voltages to output codes using the reference voltage.
