Will it not affect the frequency response?
2. The protection diodes(D3~D7) are for ESD or the feedback current of the LED

Hi Harvey,
L1 is the EVM is the common mode choke which is useful to suppress any high frequency common mode signal. It doesn't do anything to PPG signal.
I would suggest checking the datasheet of L1 for the frequency response.
D3 -D7 are for ESD.
