DLPDLCR2000EVM: Stuck pixels

We have DLPDLCR2000EVM. It worked well initially when evaluating with BBB and when testing with custom driving baseboard.
But recently, it has developed some stuck pixels - some constantly white, some black.

We have replaced the DLP chip in the optical assembly (using new DLP2000AFQC, ordered from DigiKey: 296-DLP2000AFQC-ND), but that made it much worse, exhibiting more stuck pixels and in addition to static black or white pixels, some of them are even flickering.

Using the built-in pattern generator, we have verified the issue is exhibited regardless of the signal source (all the patterns show the issue with the same pixels).

The stuck pixels are truly random ly distributed, but constant over time, there is no pattern in them that could point to signal source, connection cable issue or something of that kind.

How does this happen to the DLP chip? Could the malfunction be caused by handling during shipping? Installation? ESD?

How can we fix our EVM?
