Hard to find the free sample form.

You may already be registered as part of the TI sample program.

Can you check to see if the request samples button here appears (https://www.ti.com/product/DS90LV031A#order-quality) for the variant you want? Make sure you are logged into your myti account. 

On the order page that opens up you can click the "Add Sample to Cart" link on the bottom left hand side of the page.

You can repeat this for the DS90LV032A (https://www.ti.com/product/DS90LV032A#order-quality) once the other samples are in your cart. 

If that doesn't work, try entering a ticket to our support center (https://www.ti.com/customer-support/request-form#). Choose "Orders and Payment" and then select "Sample Program" as your topic. 

Alternatively you can reach out to the TI Sales rep for your company, they should also be able to request samples on your behalf.
