Would you be able to provide waveforms of Vsense and Vout? Also, C14, C15, C16, and C17 with values of 330nF may be too high. Could you try removing these capacitors to see if this helps?

Yes it is proper to place Rthr before the rectifier as per figure 30 in the datasheet. It seems now that the device is functioning according to the waveform posted. Does there still seem to be a problem?
