Do you have a uC in the system to use? with extra ADC and/or DAC ports?

For both cases (speaker open and short), monitoring the current would be one method.  While operating, if there is an open load, the current will be very low, if there is a short circuit, the current will be much higher, and then the outputs will turn off due to short circuit protection (at which point the current will be very low again).

Another strategy for detecting an open load during an idle or shutdown state is to use something similar to what is shown below.  In this case, SPKR_OLD_SUPPLY is a voltage source, it could be 5V, or could be from a GPIO on the microcontroller (that way you could switch between output high and Hi-Z, note that you would have to be careful with the current draw in this case).  The SPKR_OLD_LEVEL is where the ADC should go.
