20mA would be inclusive of transient peak currents, whereas 2mA is specifically for continuous.

Yes, the combined current across all pins should not exceed 20mA.

The values shown are considered to be the respective Maximum and Minimum values. For example, when the F2837x pin is outputting a logic high (VOH), it is assumed that current (IOH) will flow out from the F2837x pin to drive a load. The Maximum output drive capability is 4mA. The negative notation is from the perspective of the load. So 4mA flowing into the load is described as -4mA. There is no positive current flow provided for IOH because such a scenario would mean that the external load is actively driving energy back into the F2837x output pin, which means that there is contention, which is not supported.

You want to have a max of 20mA for all GPIOs combined. Make sure you use buffers if needed.
