The only way to do it is to examine the password locations using CCS (or) attempting to write to secure RAM. If the chip is indeed locked that may prevent you from connecting to it through CCS, unless the wait bootmode is used.
2.What causes the chip to be locked?
Hard to answer this without knowing the history of the chip. If a previous attempt to program the flash was unsuccessful (say, due to a power failure (or) the Erase/Program operation did not complete for some reason), it could leave the password locations with some random values thereby locking the device. If the error was seen on a fresh device, there could be a few reasons like a defective device or defective programming hardware.
