12V secondary winding voltage coming less 4.04V instead of 12V DC ,2.14V instead of 5V DC. 15V Secondary winding voltage is coming 14.95V Dc.

Is your own designed board? Or the TI reference board?
What input AC/DC voltage have you applied? Board's on board supply starts approximately around or above 120V.

Can you quickly check to see if 5V and/or 12V is shorted with GND (with board power off) from the test point on the board?
This will ensure that some IC or component is not shorted on this power supplies load side. It is strange because 15V rail is working fine
