The technology used for the Flash/OTP memory is fairly mature by now, with millions of devices shipped. As such, the "failure rate" is extremely low.

When the TI-issued guidelines are followed (in terms of HW & SW), the failure rate should be in single-digit PPM (from a OTP perspective)
Thanks for the clarification Hareesh. Just one small question:
we are securing Flash and RAM by configuring OTP zones. Do you anticipate any risk, if we do not test it for each and every unit?

The DCSM module, like any other module, is tested as part of our production flow. Once you have understood what security settings are warranted for your application and have configured them properly using Sysconfig and programmed them in the OTP, you only need to test one device thoroughly to ensure the security settings are what you had intended. There is no reason to test this on each and every unit.
